Terms of Service

By accessing or using IssueForm (the “Service”), you agree to be bound by these Terms of Service (“Terms”). If you do not agree, do not use the Service. The Service is operated by PIOTEE LLC (“we,” “us,” or “our”).

IssueForm is a web application that lets users generate downloadable business and legal document templates — including resumes, rental notices, HR forms, real-estate documents, finance agreements, and legal forms — as .docx and .pdf files. The Service offers a free single-document tier and a paid Pro / Bulk subscription with higher volume limits and additional template access.

IssueForm is NOT a law firm and does not provide legal advice, legal representation, or legal counsel of any kind. The document templates available through the Service are provided for informational and self-service convenience purposes only.

Use of the Service does not create an attorney-client relationship between you and PIOTEE LLC or any of its employees or agents. The templates are general in nature and may not be suitable for your specific legal situation, jurisdiction, or circumstances.

You are solely responsible for reviewing any generated document for legal accuracy, completeness, and suitability before using it. If you have questions about whether a document is appropriate for your situation, we strongly encourage you to consult a licensed attorney in your jurisdiction.

You must be at least 18 years old and capable of forming a binding contract to use the Service. By using the Service, you represent that you meet these requirements. If you are using the Service on behalf of an organization, you represent that you have the authority to bind that organization to these Terms.

Access to certain features requires signing in with a Google account. You are responsible for maintaining the security of your account and for all activity that occurs under it. Notify us immediately at hello@issueform.com if you suspect unauthorized access to your account. We are not liable for any loss resulting from unauthorized use of your account.

You agree not to:

• Use the Service for any unlawful purpose or in violation of any applicable law
• Attempt to reverse-engineer, copy, or resell the underlying templates or platform
• Use the Service to generate documents intended to deceive, defraud, or harm others
• Attempt to circumvent plan limits or rate limits
• Create multiple accounts to bypass plan restrictions
• Interfere with or disrupt the Service, its servers, or its infrastructure

The document templates provided by the Service are the intellectual property of PIOTEE LLC (see Section 9). When you fill in a template and generate a document, the completed document — including any personal or business information you entered — is yours. We do not claim ownership over the content of documents you generate.

You retain ownership of the data you enter into forms. By using the Service, you grant us a limited license to process that data solely for the purpose of rendering and delivering your requested document.

All rights in the Service — including the document templates, software, infrastructure, design, documentation, and branding — remain the exclusive property of PIOTEE LLC. We grant you a limited, personal, non-exclusive, non-transferable, non-sublicensable license to access and use the Service as permitted under these Terms. You may not copy, modify, redistribute, reverse-engineer, or create derivative works from the templates or any other part of the Service.

The Service offers a free tier that allows generation of a single document, and a paid Pro / Bulk subscription that provides increased limits and access to premium templates. Paid subscriptions are billed on a recurring basis through our payment processor, Polar.

By subscribing, you authorize us to charge your payment method on a recurring cycle until you cancel. Cancellation takes effect at the end of the current billing period; you retain access to Pro features until then. All fees are in U.S. dollars. We do not provide refunds for partial billing periods, except where required by applicable law or in cases of verified billing errors. To report a billing issue, contact hello@issueform.com within 7 days of the charge.

We reserve the right to change pricing with 30 days’ notice. Continued use of the Service after a price change constitutes acceptance of the new pricing.

The Service relies on third-party providers including Google (authentication), Polar (payment processing), and Cloudflare (hosting and infrastructure). We are not responsible for the availability, accuracy, or practices of these services. Use of third-party services is subject to their own terms and privacy policies.

THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED OR ERROR-FREE, OR THAT ANY TEMPLATE WILL BE LEGALLY SUFFICIENT FOR YOUR SPECIFIC PURPOSE OR JURISDICTION.

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, PIOTEE LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, INCLUDING LOSS OF PROFITS, REVENUE, DATA, GOODWILL, OR BUSINESS OPPORTUNITY, ARISING OUT OF YOUR USE OF OR INABILITY TO USE THE SERVICE.

OUR TOTAL CUMULATIVE LIABILITY SHALL NOT EXCEED THE GREATER OF (A) THE TOTAL AMOUNT YOU PAID TO US IN THE 12 MONTHS PRECEDING THE CLAIM OR (B) USD $100. SOME JURISDICTIONS DO NOT ALLOW CERTAIN LIMITATIONS, SO THE ABOVE MAY NOT APPLY TO YOU IN FULL.

You agree to indemnify, defend, and hold harmless PIOTEE LLC and its officers, directors, employees, and agents from any claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising out of: (a) your use of the Service; (b) your violation of these Terms; (c) any document you generate and use through the Service; or (d) any third-party claim related to content you submitted.

Either party may terminate your use of the Service at any time. We reserve the right to suspend or terminate accounts that violate these Terms, with or without notice depending on the severity of the violation. Upon termination, your access to the Service will cease and your account data will be retained and then deleted in accordance with our Privacy Policy.

We may update these Terms at any time. We will notify you of material changes by updating the effective date at the top of this page and, where appropriate, by email. Your continued use of the Service after changes constitutes acceptance of the updated Terms.

These Terms are governed by the laws of the State of Wyoming, United States, without regard to conflict of law principles. Any disputes shall be resolved in the courts located in Wyoming, and you consent to personal jurisdiction there.
